The release form nobody hands you.

A plain-English appearance release you can print, fill in and get signed — written for the case that usually goes unsigned: your own staff, on camera, in your own video.

What a release actually does

A release is one page in which the person on camera says, in writing, that you may use the footage of them. It matters later rather than now: the film is fine on the day it ships, and becomes a problem eighteen months on when the person in the opening shot has left, or the video is being cut down for an ad campaign nobody mentioned at the time. The document below covers the ordinary ground — what was recorded, where it may run, for how long, and whether it may be used to build a synthetic version of them.

Who it’s for

  • HR and internal comms teams filming staff for a culture or recruitment video
  • Marketing leads putting a founder, an engineer or a floor supervisor on camera
  • Business owners who shot something themselves and never papered it
  • Anyone who has been asked "do we have a release for this person?" and did not know

Three kinds of person end up on camera, and they are not one job.

We handle it

Talent we cast for you

Casting comes with paperwork. It is part of the job and you do not have to chase it.

You handle it

A KOL or artist you appointed

They come with their own agreement and their own rates. Their deal governs, not ours.

You handle it — internally

Your own staff

Nobody outside your company is going to push this form across the table. That is the structural reason this one is the one most often missing, and the reason this template exists.

APPEARANCE RELEASE

This release is given by the person named below ("I") to the company named below ("the Company"), for the recording described below.

1. Who, and what was recorded

I am signing this in connection with the recording described here. If more than one recording session is covered, list them.

Full name: ________________________________________

NRIC / passport no.: ________________________________________

Company: ________________________________________

Project or video: ________________________________________

Date(s) of recording: ________________________________________

2. What I am permitting

I permit the Company to record, keep and use my image, likeness, voice, name and the words I speak in that recording, and to edit that material as part of the finished work.

3. Where it may be used

The Company may use the finished work, and excerpts from it, on the channels listed below. Anything not listed is not covered by this release. Strike out what does not apply, and add what does.

☐ The Company's own website ☐ Social media accounts of the Company

☐ Paid advertising ☐ Internal use (training, induction, town halls)

☐ Trade shows, events and press ☐ Recruitment listings

☐ Other: ________________________________________

4. For how long, and where

This permission runs for the period stated below. If no period is stated, it runs until I withdraw it in writing under clause 8. Territory is worldwide unless a narrower territory is written in.

Period: ☐ Indefinite ☐ Until (date): ________________________________________

Territory: ☐ Worldwide ☐ Limited to: ________________________________________

5. Payment

Complete one. Leaving this blank is the most common cause of an argument later.

☐ No separate fee — I am recorded as part of my role with the Company.

☐ A fee of ________________________________________ has been agreed.

6. The edit

I understand the finished work is edited, and that I do not have approval over the final cut. I understand my contribution may be shortened, or may not appear at all.

7. AI and synthetic recreation — separate permission

This is a separate question from clause 2, and the default answer is no. Tick the box only if you mean it. Without a tick, the material recorded may be edited in the ordinary way but may NOT be used to train a model of, or to generate new performances by, the person signing.

☐ I permit the Company to use this material to create a synthetic or AI-generated

version of my likeness or voice, and to generate new material with it, limited to:

________________________________________

☐ I do NOT permit any synthetic or AI-generated use of my likeness or voice.

8. My personal data, and withdrawing this permission

A recording of me is personal data. The Company holds this signed release as the record of my consent, under the Personal Data Protection Act 2010. I may withdraw my permission by writing to the contact below. I understand that withdrawing it stops future use, and does not undo copies already published, printed or distributed.

Company contact for data questions: ________________________________________

This is a template, not legal advice, and we are a production studio rather than a law firm. It covers the ordinary ground and it is far better than the nothing most staff videos are shot on — but if the video carries real commercial weight, or the person on camera is a public figure, have your own lawyer read it first.
